Database of evaluations

We're creating a database of games. The database should include:

- Name of game
- ESRB rating (T, E., etc)
- Platform (check box: computer, online, Nintendo Game Cube, Playstation 2, PSP, Nintendo DS, xBox)
- if the game lab owns the game (check box yes or no)
- Required accessories (such as dance mat, bongos, etc)
- comments from kids
